Initiating Acyclovir for Herpes Zoster

Optimal Treatment Window

  • Acyclovir should ideally be started within 72 hours of rash onset for herpes zoster, but treatment initiated even between 48-72 hours still provides significant benefit in reducing pain duration and severity, as recommended by the FDA and clinical guidelines, with acyclovir accelerating cutaneous healing and reducing acute pain severity when administered within the 72-hour window 1

Special Populations Requiring Treatment

  • Immunocompromised patients should receive antiviral therapy regardless of timing, as they are at higher risk for disseminated infection, and intravenous acyclovir should be considered for complicated herpes zoster, with treatment continuing for a minimum of 7-10 days 2

Initiation of Aciclovir Therapy in Herpes Zoster

Standard Treatment Guidelines

  • The Kidney International recommends that treatment should be continued until all lesions have crusted over, not just for an arbitrary 7-day duration, for immunocompetent adults with Herpes zoster 3
  • For renal transplant patients, uncomplicated Herpes zoster should be treated with oral Aciclovir or Valaciclovir until all lesions have crusted over 3
  • Disseminated or invasive Herpes zoster requires intravenous Aciclovir with temporary reduction of immunosuppression, according to the Kidney International 3
