Chemotherapy and Immunotherapy Administration Guidelines
Patient Eligibility Criteria
- The National Comprehensive Cancer Network recommends a platelet count of ≥100,000/mm³ for safe carboplatin administration in most protocols 1
- A patient's absolute neutrophil count (ANC) of ≥1,000/mm³ is typically used as a threshold in many oncology protocols for administration of chemotherapy and immunotherapy 1
Treatment Algorithm
- Hold carboplatin and consider holding pembrolizumab if the platelet count is <100,000/mm³, as the combination regimen's efficacy depends on both agents working synergistically 2
- Recheck complete blood count in 3-7 days to assess for count recovery, and resume both agents together when platelets ≥100,000/mm³ and ANC ≥1,000/mm³ 1
Toxicity Management
- Neutropenia and thrombocytopenia are more prevalent in patients receiving chemotherapy plus immunotherapy combinations compared to chemotherapy alone, with 57% of patients experiencing treatment interruptions 2
- Growth factor support (G-CSF) should be considered for subsequent cycles to prevent recurrent neutropenia 4
