Corticosteroid Dosing for Immunosuppression

Introduction to Immunosuppression Regimens

  • The American College of Physicians recommends a corticosteroid dose of 1-2 mg/kg/day of prednisone or equivalent for moderate to severe immunosuppression, with a practical maximum of 60 mg/day 1, 2

Dosage According to Degree of Immunosuppression Required

  • For mild conditions requiring minimal immunosuppression, the recommended dosage is 0.5-1 mg/kg/day of oral prednisone 1
  • For intravenous administration in mild conditions, methylprednisolone 0.5-1 mg/kg/day IV is recommended 2
  • If no improvement is seen in 2-3 days, the dose should be increased to 2 mg/kg/day 1

Moderate to Severe Immunosuppression

  • The initial dose for moderate to severe immunosuppression should be 1-2 mg/kg/day of prednisone or an equivalent dose of methylprednisolone 1, 2
  • If no improvement is seen in 2-3 days, additional or alternative immunosuppressants should be considered 1
  • For severe conditions (grade 4), intravenous corticosteroids should be considered 2

Precautions and Special Considerations

  • Proton pump inhibitors should be started for gastrointestinal prophylaxis when using immunosuppressive doses 1, 2
  • Prophylaxis for Pneumocystis jirovecii pneumonia (PCP) should be added if more than 3 weeks of immunosuppression (>30 mg of prednisona or equivalent/day) is expected 1, 2
  • A dose of ≥20 mg/day of prednisone or equivalent for >2 weeks is considered moderate to severe immunosuppression 4, 5, 6
  • Live vaccines are contraindicated in patients with moderate to severe immunosuppression 4, 6
  • Inactivated vaccines can be administered during corticosteroid therapy, although the response may be suboptimal 4, 8

Special Situations

  • For grade 3 dermatologic toxicities, prednisone 0.5-1 mg/kg/day should be administered until the rash resolves to ≤ grade 1 1, 9
  • For severe (grade 4) toxicities, immunotherapy should be permanently discontinued and intravenous corticosteroids administered 10, 9
  • In myasthenia gravis, the goal is to find the minimum effective dose (0.3-0.6 mg/kg/day) to minimize side effects while maintaining efficacy 3
