Management of Insomnia with Mild OSA on CPAP

Patient Selection and Treatment Approach

  • The VA/DOD guidelines strongly recommend verifying patient adherence with CPAP therapy, as CPAP remains the gold standard for moderate-to-severe OSA, and should be used for the entirety of sleep periods, with educational, behavioral, and supportive interventions to improve PAP adherence in patients with OSA and concurrent insomnia 1, 2
  • The VA/DOD guidelines recognize that patients with OSA and concurrent insomnia are at high risk for poor PAP adherence, and recommend continued CPAP use as the primary OSA treatment, with lemborexant as a safe adjunctive therapy for insomnia that does not compromise respiratory function 1, 2
  • The American Academy of Sleep Medicine is not cited for any specific guideline in this article, however the VA/DOD guidelines strongly recommend PAP therapy continuation even if used <4 hours/night, and that CPAP should not be discontinued when starting insomnia medication 1, 2

CPAP Adherence and Insomnia Treatment

  • The VA/DOD guidelines recommend offering educational, behavioral, and supportive interventions to patients with OSA and insomnia to improve PAP adherence, and to continue CPAP therapy throughout treatment, as the combination addresses both conditions 1

Use of Dayvigo in Patients with Obstructive Sleep Apnea

Diagnosis and Treatment Considerations

  • The American Academy of Sleep Medicine recommends confirming OSA diagnosis with polysomnography or home sleep apnea testing, and determining if the patient is on CPAP therapy, which remains the gold standard for moderate-to-severe OSA 3, 4

Safety and Efficacy of Dayvigo

  • The American Academy of Sleep Medicine suggests that lemborexant has demonstrated respiratory safety in dedicated OSA trials, with no significant worsening of apnea-hypopnea index or oxygen saturation 3, 4

Clinical Algorithm for Prescribing Dayvigo

  • The American Academy of Sleep Medicine recommends assessing OSA severity and treatment status, evaluating for contraindications, initiating at an appropriate dose, and monitoring for respiratory effects 3, 4
