Adverse Effects of Jaypirca (Pirtobrutinib)

Neuropsychiatric Effects

  • Ibrutinib and other covalent BTK inhibitors have not been documented to cause hallucinations in the pharmacovigilance literature 1
  • Other anticancer agents like topiramate can cause hallucinations, but this is drug-specific, not a class effect 2

Clinical Considerations for Hallucinations

  • If a patient on pirtobrutinib develops hallucinations, a medication review should be conducted to screen for anticholinergics, steroids, dopaminergic agents, benzodiazepines, and other psychoactive medications that commonly cause hallucinations 3
  • Metabolic derangements such as electrolyte imbalances, hypoxia, sepsis, or poorly controlled pain should be evaluated as potential causes of hallucinations 5
  • Vision impairment should be assessed for Charles Bonnet syndrome if any degree of vision loss exists 6
  • Concurrent malignancy effects such as CNS involvement, paraneoplastic syndromes, or delirium from underlying disease should be considered as potential causes of hallucinations 5
