Diltiazem Dosing for Atrial Fibrillation with Rapid Ventricular Response
Initial Administration and Monitoring
- The American College of Cardiology recommends monitoring heart rate and blood pressure continuously during diltiazem administration for atrial fibrillation with rapid ventricular response, with a target heart rate of 60-80 beats per minute at rest 1
- The infusion rate of diltiazem may be increased in 5 mg/hour increments up to 15 mg/hour as needed for heart rate control, according to the American Heart Association 1
Precautions and Contraindications
- The American Heart Association advises avoiding diltiazem in patients with heart failure with reduced ejection fraction (LVEF ≤40%) due to negative inotropic effects 2
- The European Society of Cardiology recommends avoiding diltiazem in patients with AV block greater than first degree or SA node dysfunction (in absence of pacemaker), as well as in patients with hypotension or Wolff-Parkinson-White (WPW) syndrome with AF/atrial flutter 3, 4
Alternative Agents
- The American College of Cardiology recommends using beta-blockers (metoprolol, esmolol, propranolol) as first-line agents in patients with heart failure with reduced ejection fraction, with digoxin as an alternative for patients with heart failure 2, 5
- The American Heart Association suggests using amiodarone for patients with hemodynamic instability or severely reduced LVEF 2
Special Considerations
- For patients with thyrotoxicosis and AF, the European Heart Association recommends a beta blocker as first-line therapy, with diltiazem as an alternative when beta blockers are contraindicated 4
