Exercise-Induced Bronchoconstriction Diagnosis and Management

Diagnostic Approach

  • The American Academy of Allergy, Asthma, and Immunology recommends diagnosing EIB through objective bronchoprovocation testing, and never relying on symptoms alone, and treating with pre-exercise short-acting β2-agonists as first-line therapy, reserving daily inhaled corticosteroids for patients with underlying asthma or inadequate control 1, 2, 3
  • Baseline spirometry should be performed before and after bronchodilator administration to establish normal lung function and exclude underlying asthma, COPD, or restrictive disease, as recommended by the American Thoracic Society 4, 2
  • A focused physical examination should be conducted to look for obesity, skeletal defects, diaphragmatic dysfunction, or signs of restrictive lung disease, according to the American College of Chest Physicians 6, 4

Treatment Strategy

  • The American Academy of Allergy, Asthma, and Immunology recommends prescribing inhaled short-acting β2-agonists 15 minutes before exercise for both protection and accelerated recovery of lung function, with strong evidence (Evidence Level A) 1, 6, 2
  • Daily inhaled corticosteroids should be added when EIB occurs in patients with underlying asthma or when symptoms indicate inadequate asthma control, as recommended by the National Asthma Education and Prevention Program 2, 3, 7
  • Leukotriene receptor antagonists can be considered as alternative daily therapy, as they do not cause tolerance and attenuate EIB in approximately 50% of patients, according to the American Academy of Allergy, Asthma, and Immunology 1, 6

Monitoring and Follow-Up

  • Regular office visits are necessary because medication effectiveness varies over time due to asthma variability, environmental conditions, exercise intensity, and tachyphylaxis, as recommended by the American College of Allergy, Asthma, and Immunology 1, 6
  • Patients who require daily β2-agonist use should be reassessed, as this suggests inadequate asthma control requiring ICS therapy, according to the National Asthma Education and Prevention Program 2, 3
