Management of Parkinson's Disease with Levodopa and Carbidopa

Nutrition and Absorption

  • Avoid administering levodopa/carbidopa with protein-rich meals, as this reduces absorption and efficacy of the medication, according to Clinical Nutrition 1
  • A protein redistribution diet (low-protein breakfast and lunch, with normal protein intake at dinner) may improve motor function and increase "ON" time, although monitoring for potential side effects such as weight loss, micronutrient deficiencies, and dyskinesias is necessary 2

Special Considerations

  • Levodopa may cause hyperhomocysteinemia, especially in older patients, those with long-standing disease, and low serum levels of vitamin B12 and folate, highlighting the need for vitamin B supplementation to maintain normal homocysteine levels 3
  • Patients with constipation (a common non-motor symptom) may benefit from fermented milk with probiotics and prebiotic fiber, in addition to increased water and fiber intake 4

Management of Parkinson's Disease with Syndopa (Levodopa/Carbidopa)

Nutrition and Medication Timing

  • For optimal absorption, administer Syndopa at least 30 minutes before meals to avoid competition with dietary proteins 5, 6
  • Consider protein redistribution diet (low-protein breakfast and lunch, with normal protein intake at dinner) to improve motor function and increase "ON" time by reducing competition between levodopa and dietary amino acids 5, 6
  • Avoid administering Syndopa with high-protein meals, as this reduces drug absorption and efficacy 5, 6

Managing Motor Fluctuations

  • Monitor for potential side effects of protein redistribution including weight loss, micronutrient deficiencies, and dyskinesias 5
  • Patients experiencing the onset or worsening of dyskinesias may require reduction in levodopa doses 5
