Butalbital-Acetaminophen-Caffeine Controlled Substance Classification and Clinical Considerations

Classification and Composition

  • As a barbiturate-containing product, butalbital-acetaminophen-caffeine falls under Schedule III controlled substance regulations, as stated by the Mayo Clinic Proceedings 1, 2

Pharmacological Properties and Risks

  • Butalbital is primarily metabolized in the liver through the cytochrome P450 system, and it can induce the CYP enzyme system, potentially affecting metabolism of other medications, according to the American Journal of Kidney Diseases 3, 4
  • When combined with other CNS depressants, butalbital has additive depressant effects on the CNS and respiratory system, as reported by the American Journal of Kidney Diseases 3

Perioperative Considerations

  • For patients on long-term butalbital, abrupt discontinuation should be avoided, and if time allows before surgery, it should be weaned over 2 weeks, as recommended by the Mayo Clinic Proceedings 1, 2
  • If unable to wean prior to surgery, butalbital should be continued perioperatively to avoid withdrawal, according to the Mayo Clinic Proceedings 2
  • Additive effects with anesthetics may cause hypotension, profound sedation, and potentially death, as stated by the Mayo Clinic Proceedings 1

Clinical Recommendations

  • Due to its controlled substance status, prescriptions for butalbital-acetaminophen-caffeine require appropriate DEA documentation and cannot be refilled as easily as non-controlled medications, as noted by the Mayo Clinic Proceedings 1
  • To minimize risk of dependence, the American family physician recommends limiting use to no more than twice weekly 5
  • The American family physician also suggests considering preventive migraine therapy if medication overuse is suspected or considered a risk 5
