Management of Cytopenias with Thrombocytosis

Diagnostic Evaluation

  • The American Society of Hematology recommends a complete blood count with differential and peripheral blood smear to assess all cell lines, mean corpuscular volume, and identify schistocytes or abnormal cells 1, 2
  • The National Comprehensive Cancer Network suggests reticulocyte count to evaluate bone marrow response—a low count suggests production defects or iron deficiency, while adequate response suggests hemolysis or bleeding 2
  • Hematologic markers such as LDH, haptoglobin, indirect bilirubin, and direct antiglobulin test (Coombs) are recommended to assess for hemolysis 3, 4, 1
  • Iron studies, including serum ferritin and transferrin saturation, are necessary to identify absolute or functional iron deficiency 2
  • Liver function tests, including GGT, should be performed, as elevated GGT with pancytopenia may indicate hepatobiliary involvement 1

Risk Stratification and Management

  • The National Comprehensive Cancer Network recommends ruling out thrombotic microangiopathy (TMA) by checking ADAMTS13 activity urgently if there is evidence of microangiopathic hemolysis 4
  • Excluding myeloproliferative neoplasm is crucial, and bone marrow aspiration and biopsy with cytogenetic analysis is essential 5, 1
  • Assessing for drug-induced cytopenias is necessary, and common culprits include ribavirin, interferon, NSAIDs, antibiotics, and chemotherapy agents 6
  • For patients with myeloproliferative neoplasm, supportive care, cytoreductive therapy, and ruxolitinib may be considered 5
  • For patients with bone marrow failure syndrome, transfusion support, infection prophylaxis, and hematopoietic growth factors may be necessary 5
  • For patients with secondary causes, discontinuing offending medications, initiating iron supplementation, and addressing underlying liver pathology may be necessary 6, 2, 1

Procedural Considerations

  • Before invasive procedures, ensuring adequate platelet counts despite thrombocytosis is necessary, and maintaining >50 × 10⁹/L if active bleeding or procedures planned is recommended 1
