First-Line Treatment for OSA in Patients with CKD

Primary Treatment Approach

  • The American Academy of Sleep Medicine strongly recommends CPAP as initial therapy for all patients diagnosed with OSA, including those with CKD, with weight loss as an essential concurrent intervention for overweight and obese patients 1, 2
  • CPAP demonstrates superior efficacy in reducing apnea-hypopnea index (AHI), arousal index, and oxygen desaturation while improving oxygen saturation 3

Initiation Strategy

  • Either auto-adjusting PAP (APAP) at home or in-laboratory PAP titration is recommended for initiating therapy in adults with OSA and no significant comorbidities 1, 2
  • Educational interventions must be provided at CPAP initiation, and behavioral and troubleshooting interventions should be given during the initial treatment period to optimize adherence 1, 2

Alternative Therapies (Second-Line)

  • The American College of Physicians suggests mandibular advancement devices (MADs) as alternatives for patients who prefer them or experience CPAP adverse effects 3
  • Custom-made dual-block MADs show the strongest evidence among oral appliances 7

Monitoring Requirements

  • Adequate follow-up with troubleshooting and monitoring of objective efficacy and usage data must occur following PAP initiation and throughout treatment 1, 2
  • Monitor for CPAP adherence, as this is critical for cardiovascular risk reduction and preventing CKD progression 4

Pharmacologic Agents

  • Pharmacologic agents as primary OSA treatments lack sufficient evidence and should not be prescribed, with the exception of tirzepatide for weight loss in appropriate candidates 3, 4
