TKI Selection for Advanced Clear Cell Renal Cell Carcinoma
First-Line Treatment Options
- The European Association of Urology recommends immune checkpoint inhibitor (ICI) combinations with TKIs as the preferred first-line treatment for advanced clear cell RCC, but if a TKI alone is chosen, axitinib, cabozantinib, pazopanib, or sunitinib should be selected based on the patient's IMDC risk group and comorbidities 1, 2
- Sunitinib 50 mg daily is a category 2A option across all risk groups, with level 1A evidence demonstrating superior progression-free survival and overall survival versus interferon-alpha 1, 3, 4
- Pazopanib 800 mg daily is a category 2A option across all risk groups, with comparable efficacy to sunitinib but a superior quality of life profile 1, 3, 5
- Cabozantinib is an alternative when ICI therapy cannot be given, with level 2A evidence in intermediate and poor-risk populations 1, 2
Second-Line Treatment Options
- Cabozantinib is the preferred agent for second-line treatment after progression on first-line ICI-based therapy, with level II, B evidence 1, 6
- Nivolumab or cabozantinib are recommended after progression on first-line VEGFR TKI therapy, both with level I, A evidence and associated with overall survival benefit 6, 7, 8
Key Distinguishing Features Between TKIs
- Pazopanib has a superior health-related quality of life compared to sunitinib, but also has an increased hepatotoxicity risk requiring liver function monitoring 3, 5
- Cabozantinib is preferred for patients with bone metastases and is a preferred second-line agent after ICI-based first-line therapy 1, 4, 6
Practical Treatment Algorithm
- The National Comprehensive Cancer Network recommends determining the IMDC risk group and assessing whether the patient can receive ICI combination therapy 1, 2, 4
- If ICI is contraindicated or unavailable, the choice of TKI depends on the risk group, with cabozantinib preferred for intermediate and poor-risk patients, and sunitinib or pazopanib for favorable risk patients 1, 2, 3, 5
