Follow-Up Protocol for Bladder Villous Adenoma with Malignancy

Risk Stratification and Surveillance

  • For patients with villous adenoma and in situ adenocarcinoma, the National Comprehensive Cancer Network recommends follow-up cystoscopy every 3-6 months for the first 2 years, then annually for 5 years 1, 2
  • For patients with villous adenoma and infiltrating adenocarcinoma, post-cystectomy surveillance should follow muscle-invasive bladder cancer protocols, including urine cytology, liver function tests, and imaging every 3-6 months for 2 years, then as clinically indicated 1, 2
  • For patients with high-grade urothelial carcinoma, the Annals of Oncology recommends follow-up cystoscopy and urinary cytology every 3 months for the first 1-2 years 2
  • Upper tract imaging is recommended every 1-2 years for patients with high-grade tumors, according to Praxis Medical Insights, however, this fact will not be used as there is another fact from Annals of Oncology 2

Special Considerations for High-Grade Tumors

  • The American Cancer Society is not mentioned, however, the Annals of Oncology provides guidance on the follow-up for high-grade urothelial carcinoma, which can be applied to patients with villous adenoma and coexistent urothelial carcinoma 2
