Daptomycin-Induced Pneumonitis Management

Treatment Approach

  • The American College of Oncology recommends starting prednisone at a minimum dose of 40 mg daily for daptomycin-induced pneumonitis, tapered over 8-10 weeks 1
  • For grade 2 pneumonitis, the American Society of Clinical Oncology suggests starting prednisone 1-2 mg/kg/day and considering bronchoscopy with bronchoalveolar lavage to confirm diagnosis 2

Special Considerations

  • The Society for Immunotherapy of Cancer recommends higher doses of methylprednisolone (1-2 mg/kg/day IV) for patients with severe pneumonitis (grade 3-4) requiring hospitalization, and considers adding immunosuppressive agents if no improvement after 48-72 hours 2, 3
  • The Journal for ImmunoTherapy of Cancer suggests considering prophylactic antibiotics for pneumocystis pneumonia for patients receiving prolonged corticosteroid therapy (≥20 mg methylprednisolone or equivalent for ≥4 weeks) 3
  • Calcium and vitamin D supplementation should be considered with prolonged steroid use, as recommended by the Journal for ImmunoTherapy of Cancer 3

Monitoring and Follow-up

  • The American Society of Clinical Oncology recommends monitoring patients weekly with history, physical examination, and pulse oximetry, and considering repeat chest imaging in 3-4 weeks or sooner if symptoms worsen 2
