Antibiotic Selection for Acute Lymphocytic Leukemia with Moderate Renal Impairment

Antibiotic Dosing Considerations

  • Moxifloxacin requires no dose adjustment at CrCl 55 mL/min and can be administered at standard 400 mg IV/PO daily, which is a significant practical advantage in patients with fluctuating renal function 1
  • Levofloxacin requires dose reduction to 750 mg loading dose, then 250 mg every 24 hours for CrCl 50-80 mL/min, and for CrCl <50 mL/min, the dose is 500 mg loading dose, then 250 mg every 48 hours 2

Infection Management

  • Patients receiving purine analog-based chemotherapy require antibiotic prophylaxis for herpes zoster and Pneumocystis, as they are profoundly immunocompromised 3

Infusion Strategy

  • Prolonged infusions (3-4 hours) of beta-lactams, such as piperacillin-tazobactam, maximize time above MIC and are recommended for optimal pharmacodynamic target attainment 4
