Contraindications for Rowing Crew Participation

Understanding Rowing's Cardiovascular Demands

  • Rowing imposes extreme cardiovascular loads, classified as a Class IIIC sport, creating significant volume and pressure loads on the heart, with dramatic blood pressure elevations, according to the American Heart Association 1
  • The sport requires sustained intense physical exertion with repetitive high-force movements, as stated by the American College of Cardiology 2

Absolute Contraindications

  • Athletes with Marfan syndrome, Loeys-Dietz syndrome, Vascular Ehlers-Danlos syndrome, familial thoracic aortic aneurysm syndrome, unexplained aortic aneurysm, or any related aortic aneurysm disorder should not participate in rowing due to the risk of catastrophic aortic events, as recommended by the American Heart Association 2
  • Athletes with markedly dilated aorta (>45 mm) or severely dilated aorta (z-score >3.5-4) should not participate in any competitive sports, including rowing, as stated by the American College of Cardiology 4, 5, 2, 6, 7

Relative Contraindications

  • Athletes with mildly dilated aortas (z-score >2 or diameter >40 mm) may only participate in low-intensity sports (Class IA), not rowing, as recommended by the American Heart Association 3, 4
  • Athletes with surgical correction of aortic root/ascending aorta can only participate in Class IA sports without bodily collision, and rowing is contraindicated, as stated by the American College of Cardiology 3, 4, 6, 7

Additional Cardiovascular Considerations

  • Athletes with clinical expression and high-risk features of infiltrative cardiomyopathies (Fabry, sarcoidosis, amyloidosis, hemochromatosis) should not participate in rowing, as recommended by the American College of Cardiology 8
  • Athletes with certain repaired congenital heart disease face restrictions, including severe systemic RV dysfunction, severe RV outflow tract obstruction, and recurrent or uncontrolled atrial/ventricular arrhythmias, as stated by the American Heart Association 9
  • Athletes with systemic hypertension require careful evaluation, and uncontrolled hypertension is a relative contraindication for rowing, as recommended by the American Academy of Pediatrics 10
