Management of Clonazepam and Lamotrigine Overdose

Initial Assessment and Stabilization

  • The American Heart Association recommends immediately assessing airway, breathing, and circulation with continuous cardiorespiratory monitoring, obtaining IV access, performing bedside glucose testing, and establishing continuous cardiac monitoring with serial ECGs to detect conduction delays or dysrhythmias that can occur with lamotrigine toxicity 1, 2
  • Check responsiveness and activate emergency response protocols, assess breathing and pulse for less than 10 seconds, and obtain bedside glucose immediately to rule out hypoglycemia as a cause of altered mental status 3
  • Secure airway and provide bag-mask ventilation if respiratory depression is present, with readiness for endotracheal intubation if Glasgow Coma Scale ≤8 or protective airway reflexes are lost 1, 2
  • Continuously monitor respiratory rate, blood pressure, heart rate, oxygen saturation, and cardiac rhythm 4
  • Watch specifically for respiratory rate <8 breaths/min, hypotension, and wide complex tachycardia 1, 2
  • Arterial or venous blood gas should be performed if respiratory depression is present to assess for hypoxemia and hypercarbia 1, 2
  • Urine drug screen should be performed to identify co-ingestants, particularly opioids, alcohol, or other CNS depressants 1, 2
  • Serum acetaminophen and salicylate levels should be checked as part of standard overdose workup 3

Laboratory and Diagnostic Workup

  • Comprehensive metabolic panel including electrolytes, renal function, and hepatic function should be performed 4
  • Complete blood count should be performed 4

Specific Management Considerations

  • Flumazenil should NOT be routinely administered in this mixed overdose scenario due to multiple contraindications 1, 2
  • Standard supportive care with airway management and mechanical ventilation is preferred over flumazenil in mixed overdoses 1, 2

Observation and Disposition

  • Observe in healthcare setting until risk of recurrent toxicity is low and vital signs have normalized 3, 4, 5
  • Minimum observation period of 6-8 hours for benzodiazepine overdose, with longer periods if CNS depression persists 5
  • Monitor for at least 2 hours after any intervention to assess for recurrent toxicity 5
  • Respiratory depression requiring mechanical ventilation is a criterion for ICU admission 1, 2
  • Mandatory psychiatric evaluation before discharge to assess suicide risk 4
