Glucocorticoid Regimens for Lupus Nephritis

  • The KDIGO guidelines recommend initial glucocorticoid doses of 0.5-1.0 mg/kg/day oral prednisone or IV pulses of 250-500mg methylprednisolone for lupus nephritis treatment, always combined with immunosuppressive agents, as monotherapy with 16mg methylprednisolone is insufficient 1, 2
  • High-intensity regimen: Methylprednisolone IV pulses of 250-500mg/day for up to 3 days, followed by oral prednisone 0.8-1.0 mg/kg/day, tapering to 5mg or less by week 25, according to the KDIGO guidelines 1
  • Moderate-intensity regimen: Methylprednisolone IV pulses of 250-500mg/day for up to 3 days, followed by oral prednisone 0.6-0.7 mg/kg/day, tapering to less than 5mg by week 25, as recommended by the KDIGO guidelines 1
  • Reduced-intensity regimen: Oral prednisone 0.5-0.6 mg/kg/day, tapering to less than 2.5mg by week 25, as suggested by the KDIGO guidelines 1

Mandatory Combination Therapy

  • Glucocorticoids must be combined with immunosuppressive agents like mycophenolic acid analogs (target dose 3g/day for 6 months) or low-dose IV cyclophosphamide (total 3g over 3 months), according to the KDIGO and EULAR/ERA-EDTA guidelines 1, 2, 4
  • Belimumab plus either MPA or cyclophosphamide is also a recommended combination, as stated in the KDIGO guidelines 1, 5
  • MPA plus calcineurin inhibitor is recommended when eGFR is greater than 45 ml/min/1.73m², according to the KDIGO guidelines 1

Evidence Supporting Higher Doses

  • The EULAR/ERA-EDTA guidelines emphasize that IV methylprednisolone pulses (500-750mg) are recommended to decrease cumulative glucocorticoid exposure and associated toxicity, with a strength of evidence rated as high 4, 6

Critical Pitfalls to Avoid

  • Monotherapy failure: Using 16mg methylprednisolone without immunosuppressive agents will result in inadequate disease control and increased flare risk, with a strength of evidence rated as high 4, 6
  • Premature dose reduction: Tapering to 16mg before week 5-10 increases relapse risk, as stated in the KDIGO guidelines 1
  • Ignoring disease severity: Patients with adverse prognostic factors require higher initial doses (0.7-1.0 mg/kg/day oral prednisone or IV pulses), according to the EULAR/ERA-EDTA guidelines 4, 8

Maintenance Phase Considerations

  • Maintenance requires mycophenolic acid at reduced doses (target 2g/day) for at least 3 years, and low-dose prednisone (5-7.5mg/day), not 16mg, as recommended by the KDIGO guidelines 2, 4
  • Gradual withdrawal of glucocorticoids first, before stopping immunosuppressives, is recommended, according to the EULAR/ERA-EDTA guidelines 9
