Losartan Therapy in Proteinuric CKD Patients

Initial Dosing and Monitoring

  • The American College of Cardiology and Kidney International recommend starting with losartan 50 mg once daily as the initial dose for proteinuric CKD patients, with monitoring of serum creatinine, potassium, and blood pressure within 2-4 weeks of initiation, and uptitration to 100 mg daily if proteinuria reduction is inadequate and the medication is well tolerated 1
  • The Kidney International guideline suggests uptitrating to 100 mg daily after 4-8 weeks if proteinuria remains above target levels and the initial dose is well tolerated 1, 2

Monitoring Protocol

  • The Kidney International guideline recommends checking serum creatinine, eGFR, and potassium levels within 2-4 weeks of initiation, and measuring blood pressure to ensure adequate control without hypotension 1, 3
  • A temporary increase in serum creatinine up to 30% is acceptable and not a reason to discontinue therapy, according to the Kidney International and Praxis Medical Insights guidelines 1, 4

Dose Adjustments and Safety Considerations

  • The Kidney International guideline recommends continuing losartan even when eGFR falls below 30 ml/min per 1.73 m² unless not tolerated, and considering reducing the dose or discontinuing losartan if serum creatinine rises by more than 30% within 4 weeks of initiation, or if uncontrolled hyperkalemia or symptomatic hypotension occurs 1, 2
  • The Annals of Internal Medicine guideline recommends targeting blood pressure of ≤130/80 mmHg in proteinuric CKD patients 6, 3

Special Considerations

  • The Kidney International guideline suggests restricting dietary sodium to <2.0 g/day (<90 mmol/day) to enhance antiproteinuric effect, and considering adding a diuretic to manage edema in patients with severe proteinuria (>3.5 g/day) 7, 2
  • The Kidney International guideline recommends instructing patients to temporarily hold losartan during periods of volume depletion (acute illness with vomiting/diarrhea, excessive diuresis) 1, 2
