Severe Pain Management in Hydrocodone-Allergic Patients
Immediate Management Algorithm
- The National Comprehensive Cancer Network recommends initiating parenteral morphine 2-5 mg IV or subcutaneous for opioid-naïve patients with severe pain, or oral immediate-release morphine 5-15 mg if the patient can tolerate oral administration 1
- The American Society of Clinical Oncology suggests using hydromorphone IV/SC as an alternative to morphine, with a dose of 1/7.5 the oral morphine equivalent dose 2
- Fentanyl IV is recommended for patients requiring rapid titration, though transdermal formulations should be avoided initially, as per the National Comprehensive Cancer Network 1
Critical Titration Protocol
- The European Society for Medical Oncology recommends providing around-the-clock dosing with breakthrough doses equivalent to 10% of total daily dose for transient pain exacerbations 3
- Short half-life opioid agonists, such as morphine, hydromorphone, oxycodone, and fentanyl, are preferred for titration due to their ease of use, as suggested by the National Comprehensive Cancer Network 1
Adjunctive Non-Opioid Therapy
- The American Society of Clinical Oncology recommends combining opioids with acetaminophen/paracetamol (up to 4-6 grams daily) or NSAIDs, unless contraindicated, to enhance analgesia and potentially reduce opioid requirements 2
- Paracetamol and/or NSAIDs are effective for all intensities of pain, at least in the short term, according to the European Society for Medical Oncology 5
Alternative Strong Opioids to Avoid Cross-Reactivity
- Morphine, with a different metabolism than hydrocodone, is a suitable alternative, as recommended by the American Society of Clinical Oncology 2
- Fentanyl, with a structurally distinct phenylpiperidine class, is another alternative, as suggested by the National Comprehensive Cancer Network 1
Mandatory Monitoring
- The European Society for Medical Oncology recommends assessing pain intensity at every contact using visual analog scales, numerical rating scales, or verbal rating scales 5
- Monitoring for respiratory depression, especially during initial titration, is crucial, as per the National Comprehensive Cancer Network 1
