Metronidazole Use in Chronic Kidney Disease

Clinical Applications

  • For mild to moderate Clostridium difficile infection (CDI) in CKD patients not on dialysis, metronidazole remains an appropriate first-line therapy, with a standard dose of 500mg orally three times daily for 10-14 days, as recommended by the American Medical Association, based on evidence from JAMA 1
  • The Infectious Diseases Society of America recommends vancomycin over metronidazole for patients with severe or complicated CDI, regardless of renal function, as stated in a study published in JAMA 1

Safety Precautions

  • The National Kidney Foundation advises avoiding concomitant use of metronidazole with nephrotoxic drugs in CKD patients to prevent further kidney damage, according to a study published in the International Journal of Oral Science 2
  • The American Journal of Kidney Diseases suggests metronidazole should be temporarily suspended during intercurrent illness or planned IV radiocontrast administration, to minimize potential risks 3
