Evaluation for Meningitis
Immediate Assessment and Stabilization
- The American College of Emergency Physicians recommends that all patients with suspected meningitis require immediate hospital admission via emergency ambulance with a goal arrival time within one hour of initial assessment 1
- The Infectious Diseases Society of America suggests stabilizing airway, breathing, and circulation first, then documenting Glasgow Coma Scale (GCS) score to assess severity and monitor for deterioration 2
- The Centers for Disease Control and Prevention recommend obtaining blood cultures within the first hour of hospital arrival, before antibiotic administration 2
- The Journal of Infection guidelines state that documenting presence or absence of headache, altered mental status, neck stiffness, fever, rash, seizures, and signs of shock is crucial 1
Lumbar Puncture Decision Algorithm
- The Clinical Microbiology and Infection guidelines recommend proceeding with lumbar puncture immediately if no contraindications are present, the patient is alert and oriented, and there are no focal neurological deficits 3
- The Praxis Medical Insights guidelines state that absolute contraindications to immediate lumbar puncture include focal neurological signs, abnormal pupils, papilledema, continuous or uncontrolled seizures, GCS ≤ 12, immunocompromised state, history of CNS disease, and new onset seizure within 1 week 2
Empiric Antibiotic Therapy
- The Infectious Diseases Society of America recommends administering antibiotics within 1 hour of hospital arrival, after lumbar puncture if no contraindications exist, or immediately after blood cultures if lumbar puncture is delayed 2
- The Praxis Medical Insights guidelines suggest that empiric regimens for adults <60 years include ceftriaxone 2g IV every 12 hours or cefotaxime 2g IV every 6 hours 4
CSF Analysis
- The Clinical Microbiology and Infection guidelines recommend performing Gram stain, cell count and differential, glucose and protein levels, and bacterial culture on cerebrospinal fluid 2
- The Praxis Medical Insights guidelines state that if antibiotics are given before lumbar puncture, it should be performed within 4 hours of antibiotic initiation when possible to minimize impact on CSF culture results 2
Isolation and Infection Control
- The Centers for Disease Control and Prevention recommend placing all patients with suspected meningitis in respiratory isolation with droplet precautions until meningococcal disease is excluded 5
- The Praxis Medical Insights guidelines suggest that isolation can be discontinued after 24 hours of effective antibiotic therapy 5
Special Considerations
- The Clinical Microbiology and Infection guidelines recommend that half of adults with bacterial meningitis develop focal neurologic deficits during their clinical course, and one-third develop hemodynamic or respiratory insufficiency 3
- The Praxis Medical Insights guidelines state that common complications include cerebral infarctions, seizures, hydrocephalus, hearing loss, and cognitive deficits 3
