LDL Cholesterol Targets for TIA Patients

Current Guideline Recommendations

  • The World Stroke Organization recommends a target LDL-cholesterol level of <1.8 mmol/L (70 mg/dL) for patients with ischemic stroke and TIA, across all resource settings, as stated in the 2023 guidelines 1
  • The American Heart Association and American Stroke Association previously recommended LDL-C <100 mg/dL (2.6 mmol/L) for patients with CHD or symptomatic atherosclerotic disease, with <70 mg/dL (1.8 mmol/L) for very-high-risk persons with multiple risk factors, according to the 2006 guidelines 2, 3

Treatment Approach

  • The World Stroke Organization guidelines recommend initiating atorvastatin 80 mg daily for patients with recent TIA and LDL-cholesterol >2.5 mmol/L (>100 mg/dL) without proven cardioembolic mechanism 1
  • Adding ezetimibe 10 mg to the statin regimen is recommended if the target of <1.8 mmol/L is not achieved with statin monotherapy, as per the World Stroke Organization guidelines 1
  • For patients still not reaching target on maximally tolerated statin plus ezetimibe, consideration of PCSK9 inhibitor referral to a lipid specialist is recommended, according to the World Stroke Organization guidelines 1

Monitoring and Special Considerations

  • Lipid levels should be checked 1-3 months after treatment initiation to assess response, and continue monitoring and dose adjustments every 3-12 months thereafter, as recommended by the World Stroke Organization guidelines 1
  • For patients with TIA and documented atherosclerotic disease of extracranial or intracranial arteries, the target remains <1.8 mmol/L (70 mg/dL), with ezetimibe addition recommended to achieve this goal, according to the World Stroke Organization guidelines 1
