Intravenous Labetalol Dosing Guidelines

Maximum Cumulative Dose

  • The American Heart Association (AHA) sets the FDA‑approved ceiling for intravenous labetalol at 300 mg total within any 24‑hour period, regardless of administration method【1】.

Intermittent Bolus Dosing

  • The AHA recommends an initial bolus of 10–20 mg IV over 1–2 minutes, which may be repeated or doubled every 10–20 minutes as needed, with the overall 24‑hour cumulative limit of 300 mg【2】.

Continuous Infusion Dosing

  • The AHA advises starting a continuous infusion at 0.4–1.0 mg/kg/h (equivalent to 2–8 mg/min) and titrating up to 3 mg/kg/h, while still respecting the 300 mg/24‑h cumulative maximum【1】【3】.

Context‑Specific Dosing

Acute Ischemic Stroke – Pre‑thrombolysis

  • For patients with blood pressure > 185/110 mmHg, give 10–20 mg IV over 1–2 minutes, repeat once if target pressure is not achieved; total dose must stay within the 300 mg ceiling【2】.

Acute Ischemic Stroke – During/Post‑thrombolysis

  • Administer 10 mg IV boluses every 10–20 minutes, not exceeding 300 mg cumulative; an alternative regimen is 10 mg IV followed by an infusion of 2–8 mg/min【2】.

Hypertensive Emergency (General)

  • Give 0.3–1.0 mg/kg (maximum 20 mg) IV every 10 minutes, or start an infusion at 0.4–1.0 mg/kg/h titrated up to 3 mg/kg/h; the 300 mg cumulative dose may be repeated every 4–6 hours【1】.

Pregnancy / Pre‑eclampsia

  • The European Society of Cardiology (ESC) recommends 10–20 mg IV over 1–2 minutes, repeatable every 10–30 minutes, with a maximum single dose of 80 mg and a cumulative limit of 300 mg【4】.

Contraindications

  • According to the AHA and ESC, labetalol should not be used in patients with second‑ or third‑degree AV block, severe bradycardia, decompensated heart failure, advanced aortic stenosis, and should be used with caution in reactive airway disease or COPD【1】【4】.

Monitoring Protocol

  • The American Stroke Association (ASA) advises blood‑pressure monitoring every 15 minutes for the first 2 hours, every 30 minutes for the next 6 hours, and hourly for the subsequent 16 hours during acute intravenous labetalol therapy【2】.
