Chlorthalidone for Hypertension and Edema Management

Introduction to Chlorthalidone

  • The American College of Cardiology/American Heart Association (ACC/AHA) guidelines recommend chlorthalidone as the preferred thiazide diuretic due to its prolonged half-life and proven reduction of cardiovascular disease in landmark clinical trials 1, 2

Dosing and Administration

  • The American Academy of Pediatrics recommends starting with chlorthalidone 0.3 mg/kg per day with a maximum of 2 mg/kg per day up to 50 mg/day in children, given once daily 5
  • The initial dose for adults is 12.5 mg once daily, with titration to 25 mg once daily if blood pressure target is not achieved within 2-4 weeks 2, 4

Combination Therapy

  • The ACC/AHA guidelines recommend chlorthalidone as useful add-on therapy with drugs from other classes, making it ideal for combination with ACE inhibitors, ARBs, or calcium channel blockers 5, 3
  • For Stage 2 hypertension, initiate combination therapy immediately with two agents from different classes, such as telmisartan 40 mg plus chlorthalidone 12.5 mg once daily 3

Special Populations

  • In patients with advanced chronic kidney disease (eGFR <30 mL/min/1.73 m²), chlorthalidone is specifically superior to hydrochlorothiazide in reducing 24-hour ambulatory blood pressure 2
  • Chlorthalidone is effective as first-line therapy in diabetic patients, though it may cause small increases in fasting glucose 1

Monitoring and Adverse Effects

  • The ACC/AHA guidelines recommend monitoring serum potassium, sodium, creatinine/eGFR, uric acid, and fasting glucose in patients taking chlorthalidone 2, 3, 1
  • Hypokalemia is the most significant concern, with risk increasing with dose and duration of therapy, and can contribute to ventricular ectopy and possible sudden death 2, 4

Clinical Pitfalls to Avoid

  • Do not combine chlorthalidone with potassium-sparing diuretics when also using ACE inhibitors or ARBs, as this may cause severe hyperkalemia 5
  • Do not assume chlorthalidone and hydrochlorothiazide are interchangeable, as chlorthalidone has superior cardiovascular outcomes and longer duration of action 2
