Management of Recurrent Community-Acquired Pneumonia in Children
Hospitalization and Initial Management
- The American Academy of Pediatrics recommends hospitalization for infants less than 3-6 months and for children with recurrent disease, as this child is only 12 months old with recurrent pneumonia 1, 2
- The Infectious Diseases Society of America suggests that children with suspected bacterial CAP who cannot be reliably followed or have concerns about home observation should be hospitalized 1, 2, 3
- The American College of Chest Physicians recommends assessing for ICU admission criteria at presentation, including SpO2 ≤92% on FiO2 ≥0.50, invasive ventilation needed, or noninvasive positive pressure ventilation required 1, 2, 3, 4
Diagnostic Evaluation
- The American Thoracic Society recommends obtaining blood cultures immediately as this child requires hospitalization for presumed moderate-to-severe bacterial CAP 1, 2, 3
- Blood cultures are particularly important given the recurrent nature and need to identify specific pathogens 1, 2
- Chest radiography is recommended as a first-tier non-invasive test to establish etiologic diagnosis 7
Antibiotic Therapy
- The Pediatric Infectious Diseases Society recommends prescribing amoxicillin 45 mg/kg/day divided every 12 hours for presumed bacterial CAP 8
- Alternative consideration is amoxicillin-clavulanate at 90 mg/kg/day divided twice daily for broader coverage if β-lactamase-producing organisms or polymicrobial infection suspected 8
- If CA-MRSA suspected, consider adding clindamycin or vancomycin 2, 3, 8
