Management of Hypotension in Primary Care Setting
Initial Assessment and Determining Hemodynamic Status
- The British Journal of Anaesthesia recommends confirming the blood pressure reading and assessing for symptoms of hypoperfusion, such as dizziness, lightheadedness, and altered mental status, in a patient with hypotension 1
- Checking other vital signs, including heart rate, respiratory rate, oxygen saturation, and temperature, helps determine the cause and severity of hypotension 1
- A focused clinical examination is necessary to determine if the patient is stable or unstable, with unstable hypotension requiring immediate intervention and possible transfer to a higher level of care 1
Diagnostic Approach
- The British Journal of Anaesthesia suggests performing a passive leg raise (PLR) test to assess fluid responsiveness, which predicts whether the patient will respond to fluid administration 1, 2
- A positive PLR test indicates hypovolemia as the cause and suggests that fluid administration would be beneficial 1
Management Based on Etiology
- For fluid-responsive hypotension, the Critical Care journal recommends administering intravenous fluids, such as crystalloid solutions, starting with a 500ml bolus and reassessing response 1, 2, 3
- Avoiding hypotonic solutions, especially in patients with potential neurological issues, and restricting colloid use due to potential adverse effects on hemostasis are also recommended 3
- For non-fluid-responsive hypotension, considering vasopressors, such as norepinephrine, and dobutamine for patients with myocardial dysfunction is recommended 3
Target Blood Pressure and Monitoring
- The Critical Care journal recommends aiming for a systolic blood pressure of 80-90 mmHg until major bleeding has been controlled, and maintaining a mean arterial pressure ≥80 mmHg for patients with combined hemorrhagic shock and severe traumatic brain injury 4
- Continuous monitoring of vital signs is essential until the patient is stabilized, with regular reassessment of the patient's response to interventions 1
