Daunorubicin‑Based Salvage After Failed FLAG‑IDA

Regimen Selection (ELN 2017)

  • The 2017 European LeukemiaNet (ELN) guidelines recommend intermediate‑dose cytarabine (IDAC) combined with an anthracycline as the preferred salvage for patients who fail first‑line induction or who relapse but remain candidates for intensive therapy【1】【2】【3】.
  • Compared with repeating FLAG‑IDA, the IDAC‑anthracycline approach yields similar overall survival because FLAG‑IDA, while producing lower relapse rates, is associated with more deaths in remission【4】【5】【6】.

Age‑Based Dosing

Patients ≤ 60 years

  • IDAC: 1000–1500 mg/m² IV over 3 h every 12 h on days 1–3 (alternative: same dose over 3 h on days 1–5 or 1–6)【1】【2】【7】.
  • Daunorubicin: 45–60 mg/m² IV on days 1–3 (alternative schedule not specified)【1】【2】【8】.

Patients > 60 years

  • IDAC: 500–1000 mg/m² IV over 3 h every 12 h on days 1–3 (alternative: same dose over 3 h on days 1–5 or 1–6)【1】【2】【7】.
  • Daunorubicin: 45 mg/m² IV on days 1–3【1】【2】.

Dose‑Response Considerations

  • Cytarabine doses above 1500 mg/m² do not increase therapeutic effect, indicating a plateau beyond this level【7】【9】.

Cardiac Safety (ELN 2017)

  • When cumulative prior daunorubicin exposure is ≥ 550 mg/m², the risk of acute congestive heart failure rises markedly; alternative anthracyclines such as idarubicin 8–10 mg/m² IV on days 3–5 or mitoxantrone 8–10 mg/m² IV on days 1–3 should be considered【1】【2】【8】.

Supportive Care (NCCN 2017)

  • Growth factor: Administer G‑CSF 5 µg/kg subcutaneously starting 7 days after chemotherapy completion and continue until the white‑blood‑cell count exceeds 500 µL⁻¹【1】【2】【9】.
  • Infection prophylaxis: Provide antibacterial, antifungal, and antiviral prophylaxis throughout periods of neutropenia【10】.
  • Tumor‑lysis prophylaxis: Use allopurinol or rasburicase for patients with high blast counts【10】.
  • Transfusion thresholds: Maintain platelet counts > 10,000/µL (or > 50,000/µL if bleeding) and hemoglobin > 7–8 g/dL【10】.

Cytarabine Resistance Consideration

  • Single‑agent IDAC should be avoided in patients who relapse within 6 months after consolidation with high‑dose cytarabine, as these cases demonstrate cytarabine resistance and require alternative salvage strategies【7】【9】.

Optimizing Anthracycline Dosing in Salvage Therapy for Adult AML

Daunorubicin Dose‑Response in Younger Adults

  • In patients ≤ 60 years, a daunorubicin dose of 60 mg/m² (days 1–3) achieves comparable remission efficacy to the higher 90 mg/m² regimen while reducing 60‑day mortality (5 % vs 10 %) according to the National Comprehensive Cancer Network (NCCN) data. 11

Daunorubicin Dose‑Response in Older Adults

  • For patients > 60 years, escalating daunorubicin to 90 mg/m² does not improve overall survival and is associated with increased early mortality, supporting the use of the standard 45 mg/m² dose in this age group (NCCN). 11

Substituting Idarubicin for High Cumulative Daunorubicin Exposure

  • When cumulative prior daunorubicin exposure reaches ≥ 550 mg/m², replacing it with idarubicin 12 mg/m² administered over 3 days yields remission rates that are comparable or superior to standard‑dose daunorubicin and equivalent to dose‑intense daunorubicin (90 mg/m²) (NCCN). 11
