Consequences of Abrupt HRT Discontinuation

Introduction to HRT Discontinuation

  • The primary consequence of abruptly stopping HRT is the return of vasomotor symptoms, which occurs in approximately 50-75% of women who discontinue therapy, according to the American College of Obstetricians and Gynecologists 1

Impact on Bone Health

  • Withdrawal of HRT leads to resumption of normal postmenopausal bone loss rates, not accelerated loss, as stated by the National Osteoporosis Foundation 2
  • Bone density returns to the same rate of decline as would occur naturally in postmenopausal women without HRT, according to the National Osteoporosis Foundation 2
  • The bone protection effect of HRT is temporary and proportional to duration of treatment, as reported by the National Osteoporosis Foundation 2

Cardiovascular Considerations

  • The American Heart Association explicitly states HRT should not be used for secondary prevention of cardiovascular disease, due to lack of evidence-based benefits 3
  • HRT should never be initiated or continued for cardiovascular disease prevention, as randomized controlled trials demonstrated no benefit and potential harm, according to the American College of Cardiology 3, 4, 5, 6
  • Discontinuing HRT does not increase cardiovascular risk, as the patient was never receiving cardiovascular protection from the therapy, as stated by the American Heart Association 3, 4, 5, 6

Brain Health Claims

  • Current guidelines do not support HRT use for cognitive or brain health benefits, according to the American Academy of Neurology 1
  • No evidence-based "brain health" benefits exist that would be lost upon discontinuation, as reported by the American Academy of Neurology 1

Thrombotic Risk Reduction

  • Discontinuing HRT actually reduces thrombotic risk, which is an important benefit of stopping therapy, as stated by the American College of Chest Physicians 3, 5
  • HRT increases venous thromboembolism risk nearly 3-fold during use, according to the American College of Chest Physicians 3
  • Stopping HRT eliminates this excess thrombotic risk, as reported by the American College of Chest Physicians 3, 5

Critical Clinical Pitfall

  • HRT is indicated only for management of bothersome vasomotor and genitourinary symptoms, not chronic disease prevention, as stated by the U.S. Preventive Services Task Force 1, 7
  • The U.S. Preventive Services Task Force gives a Grade D recommendation against routine HRT use for chronic disease prevention, due to harmful effects 1
  • For every 10,000 women taking estrogen-progestin for 1 year, there are 7 additional CHD events, 8 more strokes, 8 more pulmonary emboli, and 8 more invasive breast cancers, as reported by the U.S. Preventive Services Task Force 1, 8

What to Expect After Discontinuation

  • Bone loss resumes at normal postmenopausal rates immediately after discontinuation, according to the National Osteoporosis Foundation 2
  • Thrombotic risk decreases immediately upon cessation, as stated by the American College of Chest Physicians 3
  • Breast cancer risk begins to decline after discontinuation, according to the American Cancer Society 1
