Interpreting NT-proBNP in Sepsis

Introduction to NT-proBNP Interpretation

  • The American Heart Association suggests that a markedly elevated NT-proBNP level, such as 29,332 pg/mL, in an 87-year-old female patient with sepsis likely reflects sepsis-induced cardiac stress rather than primary heart failure, especially given her normal creatinine and good left ventricular function 1, 2
  • For patients over 75 years, the diagnostic threshold for heart failure is 1,800 pg/mL, but values exceeding this cutoff, such as the patient's level, may indicate other conditions, according to the American College of Cardiology 2, 3
  • Age alone cannot explain extreme NT-proBNP elevation, as the patient's value far exceeds age-adjusted norms, as stated by the American Heart Association 2

Management Considerations

  • The American College of Cardiology recommends using NT-proBNP trends rather than absolute values to guide therapy and assess prognosis in septic patients 1
  • A decrease in NT-proBNP with treatment would indicate improved prognosis, as suggested by clinical guidelines 4
