Simvastatin Drug Interactions and Dose Modifications

Critical Dose Restrictions

  • The American College of Cardiology recommends that simvastatin dose should not exceed 10 mg daily when combined with Diltiazem due to significant increases in simvastatin exposure through CYP3A4 inhibition 1
  • The American Heart Association suggests limiting simvastatin to 10 mg daily when used with Dronedarone, due to significant increases in both prodrug and active metabolite exposure 2, 3, 4
  • The American College of Cardiology advises not to exceed 20 mg simvastatin daily when combined with Amiodarone, as this recommendation stems from the SEARCH trial, which identified 8 cases of myopathy and 7 cases of rhabdomyolysis in patients taking simvastatin 80 mg with amiodarone versus zero cases at 20 mg (relative risk 8.8) 5, 1
  • The American Heart Association recommends that simvastatin dose should not exceed 20 mg daily when combined with Amlodipine, due to minor increases in statin exposure 1

Mechanism of Interactions

  • The American College of Cardiology states that all major simvastatin interactions involve CYP3A4 inhibition, as simvastatin is extensively metabolized through this pathway 5, 1
  • The American Heart Association notes that Amiodarone produces approximately 75% increase in simvastatin AUC and Cmax through both CYP3A4 and P-glycoprotein inhibition 5, 1

Special Populations Requiring Caution

  • The American College of Cardiology identifies patients at highest risk for myopathy, including elderly patients, particularly thin or frail women, patients with multisystem disease, those with diabetes combined with chronic renal failure, and patients of Asian descent 6
  • The American Heart Association suggests exercising particular caution with statin-calcium channel blocker combinations in patients of Asian descent 5, 1

Monitoring Requirements

  • The American College of Cardiology recommends essential monitoring parameters, including evaluating muscle symptoms at 6-12 weeks after starting therapy and at each follow-up visit, obtaining creatine kinase measurement when patients report muscle symptoms, and monitoring ALT/AST at 12 weeks after initiation, then annually 6
  • The American Heart Association advises that higher doses of simvastatin with interacting drugs may be considered only with close monitoring for muscle-related toxicity 2, 3

Preferred Alternative Statins

  • The American College of Cardiology suggests considering switching to Rosuvastatin, Pitavastatin, or Pravastatin when significant CYP3A4 inhibitors are necessary 5, 2, 3, 1
  • The American Heart Association notes that these alternative statins show reasonable safety profiles when combined with Amiodarone, Diltiazem, or Verapamil 5, 2, 1, 3

No Significant Interactions

  • The American College of Cardiology states that simvastatin can be safely combined with Warfarin, though monitoring INR more closely after statin initiation or dose changes is recommended 2, 3, 4
  • The American Heart Association suggests that coadministration of simvastatin and Digoxin is reasonable if clinically indicated 2, 3, 4
