Diagnosis and Management of Anterior Neck Mass
Initial Evaluation and Risk Stratification
- The American Academy of Otolaryngology guidelines emphasize that any neck mass in an adult should be considered malignant until proven otherwise, particularly when there is no infectious etiology 2
- A mass present for ≥2 weeks without significant fluctuation, or of uncertain duration, or with no history of recent infection, or with progressive enlargement, or accompanied by hoarseness, dysphagia, or otalgia, or unexplained weight loss, or smoking or alcohol history, should be considered high-risk for malignancy 1, 2, 5, 6
- Physical examination red flags include size >1.5 cm, firm or hard consistency, fixation to adjacent tissues, and ulceration of overlying skin 1, 2, 4
Mandatory Workup for High-Risk Patients
- The American Academy of Otolaryngology-Head and Neck Surgery recommends performing or referring for visualization of the larynx, base of tongue, and pharynx to search for a primary malignancy 1, 4, 5
- CT neck with contrast (or MRI with contrast) should be ordered immediately for any high-risk patient, as recommended by the AAO-HNS guidelines 1, 2, 4, 6, 7
- Fine-needle aspiration (FNA) should be performed instead of open biopsy when diagnosis remains uncertain after imaging, as recommended by the AAO-HNS guidelines 1, 4, 7, 8
Management of Cystic Masses
- If imaging or FNA reveals a cystic mass, evaluation should continue until diagnosis is obtained—never assume it is benign, as cystic masses can be malignant 1, 4, 8, 3
- HPV-positive oropharyngeal squamous cell carcinoma commonly presents as cystic cervical metastases, and papillary thyroid carcinoma and lymphoma can also be cystic 5, 8
What NOT to Do
- Antibiotics should NOT be routinely prescribed unless there are clear signs and symptoms of bacterial infection, as recommended by the AAO-HNS guidelines 7, 6, 3
- Open biopsy should only be done after examination under anesthesia with panendoscopy if FNA, imaging, and ancillary tests fail to identify a diagnosis or primary site 1, 4, 7, 3
