Perioperative Management of Apixaban for Orthopedic Surgery
Introduction to Apixaban Management
- The French Working Group on Perioperative Hemostasis recommends discontinuation of apixaban up to five days before surgery when accounting for patient-specific factors, such as renal function and bleeding risk 1
Preoperative Considerations
- Creatinine clearance significantly impacts apixaban elimination and must guide discontinuation timing, with calculations using the Cockcroft-Gault formula 1, 2
- Patients with creatinine clearance 30-50 mL/min require longer discontinuation periods beyond the standard 48 hours 3
- The American Society of Anesthesiologists and other guideline societies consider age ≥80 years, concomitant P-glycoprotein inhibitors, and renal impairment as factors that may require extending the discontinuation period beyond 48 hours 2
Neuraxial Anesthesia and Apixaban
- The American Society of Regional Anesthesia and Pain Medicine recommends not performing spinal or epidural anesthesia if there is any possibility of residual apixaban concentration from insufficient discontinuation time 2
- Elderly patients (>80 years) or those with renal impairment are at higher risk and require careful consideration before proceeding with neuraxial techniques 2
Bridging Anticoagulation
- The American College of Cardiology and other guideline societies recommend that preoperative bridging with heparin is not indicated for most patients, as it increases bleeding risk without reducing thrombotic complications 1, 2, 3
- Bridging is only recommended for patients at very high thrombotic risk 1
Postoperative Resumption of Apixaban
- Apixaban can be resumed when adequate hemostasis is established, typically 12-24 hours after surgery, according to the American College of Chest Physicians 4
- For immediate postoperative VTE prophylaxis, heparin or fondaparinux can be given at least 6 hours after the procedure 1, 4
- The European Society of Cardiology recommends monitoring renal function postoperatively as surgical stress may affect kidney function and apixaban dosing requirements 1, 4
