Antibiotic Regimens for Post-Neurosurgical Meningitis
Empiric Therapy
- The Infectious Diseases Society of America recommends initial empiric therapy targeting both Gram-positive and Gram-negative organisms for post-neurosurgical meningitis, with a regimen of vancomycin plus either cefepime, ceftazidime, or meropenem 1
- The recommended empiric regimen is vancomycin 15-20 mg/kg IV every 12 hours, plus one of the following: ceftazidime 2 g IV every 6 hours, cefepime 2 g IV every 8 hours, or meropenem 2 g IV every 8 hours 1, 2
Pathogen-Specific Therapy
- For methicillin-susceptible Staphylococcus aureus, the Infectious Diseases Society of America recommends nafcillin or oxacillin, with alternative options including vancomycin or meropenem 3
- For methicillin-resistant Staphylococcus aureus (MRSA), vancomycin is recommended, with consideration of adding rifampin 600 mg IV/orally every 12 hours, especially with CSF shunt infections 1, 2, 3
Duration of Therapy
- The Infectious Diseases Society of America recommends 10-14 days of antibiotic therapy for uncomplicated post-neurosurgical meningitis with good clinical response, with consideration of extension if clinical response is delayed 2, 4
Special Considerations
- The Infectious Diseases Society of America recommends removal of all components of the infected shunt in addition to appropriate antimicrobial therapy if a CSF shunt is present and infected 1
- For patients with CSF shunts infected with staphylococci, consideration of adding rifampin to vancomycin is recommended, especially if the shunt cannot be removed 1
Monitoring and Adjustments
- Regular monitoring of clinical response and CSF parameters is essential, with consideration of repeat CSF analysis if clinical improvement is not observed 2, 4
- For vancomycin, monitoring of serum trough levels to maintain 15-20 μg/mL is recommended 2, 4
Antibiotic Treatment for Post-Neurosurgical Meningitis
Pathogen-Specific Treatment
- For Streptococcus pneumoniae, the Infectious Diseases Society of America recommends penicillin-sensitive treatment with benzylpenicillin 2.4g IV every 4 hours or continue ceftriaxone/cefotaxime 5
- For Streptococcus pneumoniae, the Infectious Diseases Society of America recommends penicillin-resistant but cephalosporin-sensitive treatment with continue ceftriaxone 2g IV every 12 hours or cefotaxime 2g IV every 6 hours 5
- For Streptococcus pneumoniae, the Infectious Diseases Society of America recommends penicillin and cephalosporin-resistant treatment with ceftriaxone or cefotaxime plus vancomycin plus rifampin 600mg twice daily 6
- For Staphylococcal species, the Infectious Diseases Society of America recommends treatment for 14 days 5
- For Streptococcus pneumoniae, the Infectious Diseases Society of America recommends treatment for 10-14 days 6
