Retinol Discontinuation Before Conception

Key Recommendations

  • The retinol form of vitamin A should be avoided during pregnancy due to teratogenic risk, especially in the first trimester, and women should replace vitamin A supplements from retinol to beta-carotene form during preconception planning 1, 2, 3
  • Women planning pregnancy should transition to prenatal multivitamins containing vitamin A in beta-carotene form rather than retinol 1, 4

Special Populations

  • Women who have undergone bariatric surgery should have vitamin A levels checked at least every 3 months when planning pregnancy, and supplementation should be adjusted to maintain concentrations within normal limits using beta-carotene form 2, 3
  • For women with cystic fibrosis or other malabsorptive conditions, vitamin A intake and blood levels should be assessed before conception or early in pregnancy, with supplementation aiming to achieve normal serum retinol concentrations 4, 5

Preconception Planning

  • Begin preconception planning 3-6 months before attempting conception, switching to beta-carotene-containing supplements at this time 2
  • No specific washout period is required for standard retinol supplements, but the switch should be made before actively trying to conceive 1, 2, 3

Vitamin A and Isotretinoin Contraindication in Pregnancy

Teratogenic Effects

  • The American College of Obstetricians and Gynecologists recommends that pregnant women avoid vitamin A in retinol form due to teratogenic risk, especially in the first trimester, with daily doses higher than 8,000 IU not recommended 6, 7

Safer Alternatives

  • The National Institutes of Health suggests that beta-carotene form of vitamin A is preferred over retinol during pregnancy, with foods high in beta-carotene providing necessary amounts of vitamin A without associated toxicity or teratogenicity 6, 7
  • Many prenatal multivitamins contain vitamin A in the safer beta-carotene form, according to the Academy of Nutrition and Dietetics 8

Clinical Management

  • For women planning pregnancy, the American Academy of Dermatology recommends discontinuing isotretinoin at least 1 month before attempting conception and switching from retinol-containing supplements to those with beta-carotene form of vitamin A 7
  • For pregnant women with accidental exposure, the Centers for Disease Control and Prevention suggests a comprehensive evaluation of potential risks, including detailed ultrasound examination to detect possible malformations 6, 7
