Initial Treatment for Patients with Chronic Liver Damage Child-Pugh C

Introduction to Treatment Options

  • The American College of Radiology recommends that patients with chronic liver damage classified as Child-Pugh C be considered for liver transplantation, hormonal therapy, or supportive palliative care, depending on the patient's specific condition and organ availability 1, 2

Opciones de tratamiento según el tipo de lesión

  • For patients with a single lesion of hepatocellular carcinoma (HCC) less than 5 cm in a cirrhotic liver Child-Pugh C, the preferred treatment option is liver transplantation if the patient meets the criteria, with percutaneous techniques and hormonal therapy considered in selected cases 1, 2
  • For patients with a single lesion of HCC greater than 5 cm, hormonal therapy is recommended, with symptomatic/palliative treatment also considered 1, 3

Consideraciones importantes

  • The European Association for the Study of the Liver notes that patients with Child-Pugh C have severely compromised liver function, which significantly limits therapeutic options 4
  • The American Association for the Study of Liver Diseases recommends that liver transplantation be considered the best long-term treatment option for selected patients who meet the Milan criteria (single tumor ≤5 cm or 2-3 tumors ≤3 cm, without vascular invasion) 4
  • The European Association for the Study of the Liver advises that patients with Child-Pugh C should not receive current antiviral regimens due to the high risk of potentially fatal complications 5

Manejo de complicaciones específicas

  • For patients with Child-Pugh C and acute variceal hemorrhage, the American College of Radiology recommends endoscopic management, medical therapy with vasoactive drugs, and transjugular intrahepatic portosystemic shunt (TIPS) in selected cases 6, 7
  • For patients with Child-Pugh C and ascites, the American College of Radiology recommends dietary modification (sodium restriction), large-volume paracentesis, and volume expansion 7, 8

Algoritmo de decisión terapéutica

  • The Annals of Oncology suggests evaluating if the patient is a candidate for liver transplantation (according to Milan or UCSF criteria), and if not, assessing the presence of specific complications such as variceal hemorrhage, ascites, or encephalopathy, and managing them accordingly 4, 7, 8
