EMTALA Guidelines for STEMI Patients
Core Requirements
- The American College of Emergency Physicians recommends that all patients presenting to an emergency department must receive a medical screening examination to determine if an emergency medical condition exists, followed by necessary stabilizing treatment, regardless of ability to pay 1, 2
- The Emergency Medical Treatment and Labor Act (EMTALA) requires hospitals to provide stabilizing treatment to patients with emergency medical conditions, including STEMI patients, before transferring them to another facility 1, 2
- The American Heart Association recommends that hospitals with specialized capabilities, such as PCI-capable centers, have a responsibility to accept transfer of patients when such transfer is necessary to stabilize an emergency medical condition 3, 5, 8
Application to STEMI Patients
- For STEMI patients, EMTALA specifically requires that the rural ED perform an appropriate medical screening examination to identify the STEMI as an emergency medical condition 1, 2
- The American College of Cardiology recommends that stabilizing treatment, including aspirin, antiplatelet therapy, anticoagulation, and pain control, must be initiated within the capability of the rural hospital before transfer 6, 7
- The receiving tertiary center with specialized PCI capabilities has a responsibility to accept the transfer when such transfer is necessary to stabilize an emergency medical condition 3, 5, 8
Critical Transfer Requirements
- The American College of Emergency Physicians recommends that transfer of patient care responsibilities between physicians must be orderly, clearly defined, and properly documented 3, 4
- On-call physician services must be available within a reasonable time to provide necessary stabilizing treatment without regard to the patient's ability to pay 1, 2, 3
- Hospitals with specialized capabilities have a responsibility to accept transfer of patients when such transfer is necessary to stabilize an emergency medical condition 3, 5, 8
