Endometrial Carcinosarcoma Treatment Guidelines
Primary Surgical Management
- The European Society of Gynaecological Oncology recommends complete surgical staging, including total hysterectomy with bilateral salpingo-oophorectomy, acquisition of peritoneal fluid or washings, thorough exploration of the abdominal cavity, evaluation of pelvic and para-aortic nodal areas, and omentectomy, for the treatment of endometrial carcinosarcoma 1, 2
- Retroperitoneal lymph node dissection has therapeutic value and is essential for accurate staging, which guides adjuvant therapy decisions 2
- For advanced disease, maximal surgical cytoreduction to no residual macroscopic disease should be attempted in patients with good performance status 3
Adjuvant Treatment for All Stages
- The National Comprehensive Cancer Network recommends combined chemotherapy and radiotherapy for optimally debulked stage III-IV disease, with carboplatin/paclitaxel doublet as first-line chemotherapy 3, 4
- Cisplatin and doxorubicin combination significantly improves both progression-free survival and overall survival compared to whole abdominal radiation therapy alone 3, 4
- Combined chemoradiation reduces the risk of relapse or death by 36% and improves cancer-specific survival 4
Preoperative Staging Workup
- The American College of Radiology recommends preoperative staging workup, including chest X-ray, contrast-enhanced dynamic MRI, CT scan of abdomen and retroperitoneal nodes, blood counts, liver and renal function profiles, and transvaginal ultrasound, to assess uterine and locoregional pelvic extension 1, 2
