Narcolepsy with Cataplexy Management

Rationale for Treatment

  • The American Academy of Sleep Medicine issues strong recommendations for modafinil/armodafinil, sodium oxybate/Xywav, and pitolisant as first-line treatments for narcolepsy in adults, with moderate-quality evidence from multiple RCTs and observational studies 1, 2

Medication Mechanisms and Efficacy

  • Xywav works via GABAB receptors to treat excessive daytime sleepiness and cataplexy, with moderate-quality evidence from 6 RCTs and 6 observational studies 1
  • Pitolisant functions as a histamine H3-receptor antagonist/inverse agonist, increasing histamine synthesis and modulating release of norepinephrine and dopamine, with moderate-quality evidence from 3 RCTs 1

Safety Considerations

  • Xywav carries an FDA black box warning as a CNS depressant that may cause respiratory depression, and is a Schedule III controlled substance 1, 4
  • Armodafinil and pitolisant may reduce oral contraceptive effectiveness, and have potential for abuse, with armodafinil being a Schedule IV controlled substance 1

Monitoring and Follow-up

  • Regular follow-up is essential to assess treatment efficacy and adverse effects, with more frequent visits when starting medications or adjusting doses 3, 4
  • Monitor for adverse effects of stimulants, respiratory depression with Xywav, and signs of depression or suicidality 3, 4, 5

Special Population Considerations

  • All three agents may cause fetal harm based on animal data, with insufficient human data to determine risk, and careful monitoring is required in pregnant and breastfeeding women 1, 2
  • Elderly patients require careful monitoring due to potential adverse effects, particularly with Xywav 4
