Bicalutamide-Associated Hypercholesterolemia

Monitoring and Management

  • Regular monitoring of lipid profiles is recommended for patients on bicalutamide therapy, particularly those on combination therapy with LHRH agonists, and initial management of clinically significant hypercholesterolemia should include dietary modifications with restriction of cholesterol and saturated fat intake 1
  • When selecting lipid-lowering therapy for patients on bicalutamide, statins are generally safe and effective for managing dyslipidemia, with pravastatin being a preferred option as it is not metabolized by the same cytochrome P450 pathway (CYP3A4) that metabolizes bicalutamide, although caution should be used with lipophilic statins due to increased risk of drug interactions 1
  • Statins are generally safe and effective for managing dyslipidemia in patients on bicalutamide 2

Clinical Considerations

  • Bicalutamide has less effect on serum lipids compared to other antiandrogens like cyproterone acetate 3
  • The risk of hypercholesterolemia should be balanced against the benefits of bicalutamide therapy in controlling prostate cancer, as recommended by the National Comprehensive Cancer Network 4
