Constipation Treatment with Constella and PEG3350

Mechanism of Action and Efficacy

  • PEG3350 is an osmotic laxative that works by drawing water into the intestinal lumen, softening stool and increasing bowel movements, as noted by the American Gastroenterological Association 1, 2
  • Linaclotide (Constella) works through a different mechanism as a guanylate cyclase-C agonist, increasing intestinal fluid secretion and accelerating transit, according to the American Gastroenterological Association 3
  • The different mechanisms of action make these medications complementary when used together, as suggested by the American Gastroenterological Association 4, 2

Evidence Supporting Combination Use

  • The American Gastroenterological Association guidelines recommend PEG3350 as a first-line agent for chronic constipation with strong recommendation and moderate certainty of evidence 4, 2
  • For patients with inadequate response to a single agent, combination therapy with medications that work through different mechanisms is a logical approach, as recommended by the American Gastroenterological Association 4, 5
  • The American Gastroenterological Association suggests that combination therapy approaches are acceptable, including the use of PEG3350 in combination with other medications 2, 5

Dosing and Administration

  • PEG3350 is typically dosed at 17g once daily mixed in 8 ounces of liquid, as recommended by the American Gastroenterological Association 6
  • When using both medications, start with standard dosing of each and monitor response, as suggested by the American Gastroenterological Association 2, 6

Monitoring and Side Effects

  • When using both medications, monitor for excessive loose stools or diarrhea, as both medications can cause these side effects, according to the American Gastroenterological Association 2, 5
  • PEG3350 may cause abdominal distension, flatulence, and nausea, as noted by the American Gastroenterological Association 2, 6

Treatment Algorithm

  • The American Gastroenterological Association recommends starting with PEG3350 alone at 17g daily as first-line therapy, and adding linaclotide (Constella) if response is inadequate after 2-4 weeks 4, 2, 3, 5
  • Titrate doses of each medication based on response and side effects, as suggested by the American Gastroenterological Association 2, 6
  • Once optimal response is achieved, continue both medications at effective doses, according to the American Gastroenterological Association 5, 6

Special Considerations

  • For patients with severe constipation or those who have failed multiple single-agent therapies, combination therapy may be particularly beneficial, as recommended by the American Gastroenterological Association 4, 5
  • PEG3350 is widely available over-the-counter and relatively inexpensive, making it accessible for long-term combination use, as noted by the American Gastroenterological Association 2, 5
