Pediatric Medication Dosing Guidelines for Children Under 40 kg

General Dosing Principles

  • For children under 40 kg, including a 4-year-old male weighing 13 kg, medication dosing must be calculated using weight-based formulas (mg/kg) rather than body surface area, as recommended by the American Academy of Pediatrics, to ensure accurate dosing 1

Antibiotic Dosing

  • For mild to moderate infections, the American Academy of Pediatrics recommends amoxicillin dosing of 20-25 mg/kg/day divided every 8-12 hours, which for a 13 kg child would be approximately 260-325 mg/day 2
  • For severe infections or lower respiratory tract infections, the American Academy of Pediatrics recommends amoxicillin dosing of 40-45 mg/kg/day divided every 8-12 hours, which for a 13 kg child would be approximately 520-585 mg/day 2
  • The Centers for Disease Control and Prevention recommend isoniazid dosing of 10-15 mg/kg daily (maximum 300 mg) for tuberculosis treatment, which for a 13 kg child would be approximately 130-195 mg daily 3
  • The American Thoracic Society recommends rifampin dosing of 10-20 mg/kg daily (maximum 600 mg) for tuberculosis treatment, which for a 13 kg child would be approximately 130-260 mg daily 3
  • The American Academy of Pediatrics recommends pyrazinamide dosing of 15-30 mg/kg daily for tuberculosis treatment, which for a 13 kg child would be approximately 195-390 mg daily 3, 4

Antiviral Medication Dosing

  • The American Academy of Pediatrics recommends oseltamivir dosing of 30 mg twice daily for 5 days for influenza treatment in children weighing ≤15 kg, including a 13 kg child 2, 4, 5
  • The Centers for Disease Control and Prevention recommend oseltamivir dosing of 30 mg once daily for 10 days for chemoprophylaxis in children ≥1 year old, but note that chemoprophylaxis is only FDA-approved for ≥13 years 5, 6

Intra-Abdominal Infection Antibiotic Dosing

  • The Infectious Diseases Society of America recommends metronidazole dosing of 30-40 mg/kg/day divided every 8 hours for intra-abdominal infections, which for a 13 kg child would be approximately 390-520 mg/day 7
  • The American Academy of Pediatrics recommends ceftriaxone dosing of 50-75 mg/kg/day divided every 12-24 hours for intra-abdominal infections, which for a 13 kg child would be approximately 650-975 mg/day 7, 4
  • The Infectious Diseases Society of America recommends piperacillin-tazobactam dosing of 200-300 mg/kg/day of piperacillin component divided every 6-8 hours for intra-abdominal infections, which for a 13 kg child would be approximately 2,600-3,900 mg/day 7, 4

Metronidazole Dosing Considerations

Dosing Regimens

  • The maximum single dose of metronidazole should not exceed 500 mg 8

Special Considerations

  • In children with severe malnutrition and hepatic involvement, a dose reduction of 60% to approximately 12 mg/kg/day may be necessary, as hepatic dysfunction significantly prolongs metronidazole half-life 8
