Valproate Dosing and Administration

Status Epilepticus Management

  • The American College of Emergency Physicians recommends administering 20-30 mg/kg IV over 10 minutes for acute status epilepticus, achieving 63-88% efficacy in terminating seizures 1, 2, 3
  • The loading dose for status epilepticus is 20-30 mg/kg IV, with an infusion rate of up to 10 mg/kg/min 1, 2, 3
  • Repeat dosing of 20 mg/kg may be given after 15 minutes if seizures persist, with a maximum total loading dose of 40 mg/kg 3
  • The target therapeutic level for seizures is 50-100 μg/mL 3
  • Valproate is superior to phenytoin for status epilepticus, with 66% vs 42% efficacy and significantly less hypotension and respiratory depression 1, 2

Mood Stabilization and Monitoring

  • The American Academy of Family Physicians recommends an initial dose of 125 mg twice daily for mood stabilization, with a target therapeutic level of 40-90 μg/mL 4, 5
  • Monitoring of liver enzymes, complete blood count, and PT/PTT is essential at baseline and regularly throughout treatment 4, 5, 3
  • Serum levels should be checked within 48-72 hours after loading dose 3

Special Considerations

  • The evidence suggests that valproate has no risk of soft tissue injury from extravasation, unlike phenytoin 3
  • Enzyme-inducing antiepileptic drugs may reduce valproate half-life, requiring higher or more frequent dosing 1, 2
