Medical Necessity Determination for Aflibercept Continuation in Branch Retinal Vein Occlusion

Critical Missing Documentation

  • The American Academy of Ophthalmology recommends that continuation of anti-VEGF therapy requires demonstration of positive clinical response, defined as improvement in best corrected visual acuity (BCVA), maintenance of BCVA, reduction in macular edema on optical coherence tomography (OCT) imaging, or reduction in the rate of vision decline 1, 2
  • The insurance criteria require "improvement or maintenance in best corrected visual acuity [BCVA] or visual field, or a reduction in the rate of vision decline or the risk of more severe vision loss" for continuation of aflibercept therapy 1, 2

Guideline Requirements for Continuation Therapy

  • The American Academy of Ophthalmology Retinal Vein Occlusions Preferred Practice Pattern states that continuation of anti-VEGF therapy requires demonstration of positive clinical response, including reduction in macular edema on OCT imaging 2

Alternative Treatment Considerations

  • The American Academy of Ophthalmology suggests that intravitreal corticosteroids (dexamethasone implant or triamcinolone) may be considered for branch retinal vein occlusion (BRVO) with macular edema, though with risks of cataract and elevated intraocular pressure (IOP) 1, 2, 4
  • Grid laser photocoagulation may be considered for BRVO with macular edema and visual acuity ≤20/40, though anti-VEGF therapy is superior 2, 4

Required Documentation for Approval

  • The American Academy of Ophthalmology recommends that providers submit baseline and current best corrected visual acuity (BCVA) measurements, baseline and current optical coherence tomography (OCT) measurements, and documentation of treatment interval and response pattern to demonstrate medical necessity for aflibercept continuation 1, 2, 3

Common Pitfalls in RVO Treatment Authorization

  • Failure to document objective response metrics, such as quantitative OCT data, is the most common reason for denial of aflibercept continuation 1, 2, 3
