Diagnostic Hypocretin Level in Narcolepsy Type 1

Clinical Significance of Hypocretin Levels

  • Type 1 narcolepsy is caused by degeneration of hypothalamic neurons that produce orexin/hypocretin, according to the American Academy of Sleep Medicine 1, 2

Diagnostic Algorithm

  • First-line testing for narcolepsy involves overnight polysomnography (PSG) followed by Multiple Sleep Latency Test (MSLT), as recommended by the American Geriatrics Society 3, 4
  • Mean sleep latency ≤8 minutes and ≥2 sleep-onset REM periods on MSLT suggest narcolepsy, with a moderate level of evidence 3

Pitfalls and Caveats

  • CSF hypocretin-1 testing is not routinely recommended in Prader-Willi syndrome despite narcolepsy-like features, as levels are typically not as low as in narcolepsy type 1, according to the American Academy of Sleep Medicine 1, 2, 6
