Cerebrospinal Fluid Characteristics and Functions
Production and Volume
- The choroid plexus is the primary source of cerebrospinal fluid (CSF) production, generating approximately 800 ml during 24 hours 1
- The CSF is completely renewed about 4 times a day, replacing the total volume more than 5 times daily 1
Circulation
- The CSF flows from the lateral ventricles through the foramina of Monro into the third ventricle 1
- It continues through the aqueduct of Sylvius into the fourth ventricle 1
- The CSF exits through the foramina of Magendie and Luschka into the base of the brain 1
- From the base of the brain, the CSF travels caudally to the base of the spinal cord (lumbar sac) and rostrally over the cerebral convexities 1, 2
Composition and Functions
- The glucose level in CSF is approximately 2/3 of the serum glucose level 3
- The CSF transports nutrients to the brain and central nervous system 4
- The National Comprehensive Cancer Network recommends that at least 10-15 ml of CSF be extracted to minimize false-negative cytology results 2
Pathological Characteristics of CSF
- In viral meningitis, the CSF may present with lymphocytic pleocytosis, elevated proteins, and normal glucose levels 5
- In bacterial meningitis, the CSF may present with neutrophilic pleocytosis, elevated proteins, and low glucose levels 5
- In leptomeningeal metastases, the CSF may present with elevated opening pressure, low glucose, high proteins, and lymphocytic pleocytosis 2
- In neurodegenerative diseases such as Alzheimer's, there are alterations in specific biomarkers like beta-amyloid and tau 6
Analysis of CSF
- The gold standard for diagnosing leptomeningeal metastases is the cytological identification of malignant cells in the CSF 2
- Immediate processing and proper handling of the CSF sample are crucial 2
- In cases of suspected antibody-mediated encephalitis, the CSF may show lymphocytic pleocytosis (up to 500 cells/L) 5
- Inflammatory biomarkers in CSF can be useful for diagnosing neurodegenerative diseases 7
