Bence Jones Proteins: Diagnosis and Clinical Manifestations

Diagnostic Methods

  • The National Comprehensive Cancer Network recommends the gold standard for detecting Bence Jones proteins is 24-hour urine collection with protein electrophoresis and immunofixation 1, 2
  • The serum free light chain (FLC) ratio (kappa/lambda) provides crucial diagnostic information, with abnormal ratios indicating monoclonal light chain production, as suggested by the Clinical Cancer Research and Haematologica 2, 3
  • Active myeloma is defined as Bence Jones protein >2500 mg/24 hours, according to the National Comprehensive Cancer Network 1, 4
  • Smoldering myeloma is defined as Bence Jones protein ≥500 mg/24 hours without end-organ damage, as indicated by Clinical Cancer Research 2

Clinical Manifestations and Associated Symptoms

  • Renal insufficiency (creatinine >2 mg/dL or creatinine clearance <40 mL/min) is a cardinal feature of active myeloma with Bence Jones proteinuria, as stated by the National Comprehensive Cancer Network 1, 4
  • Anemia (hemoglobin <10 g/dL or >2 g/dL below normal) occurs due to bone marrow infiltration by plasma cells, according to the National Comprehensive Cancer Network 1, 4
  • Osteolytic bone lesions on skeletal radiography, CT, or PET-CT are a common manifestation of multiple myeloma, as indicated by the National Comprehensive Cancer Network 1, 4
  • Hypercalcemia (calcium >11 mg/dL or >1 mg/dL above upper limit of normal) from bone destruction is a frequent complication, as stated by the National Comprehensive Cancer Network 1, 4

Risk Stratification and Prognosis

  • High-risk features for progression include markedly abnormal serum FLC ratio (<0.01 or >100), as suggested by the National Comprehensive Cancer Network and Clinical Cancer Research 1, 2
  • Elevated serum creatinine is a high-risk feature for progression, according to the National Comprehensive Cancer Network 1

Monitoring Recommendations

  • The National Comprehensive Cancer Network and Praxis Medical Insights recommend performing blood and urine analysis every 3-6 months, with bone studies annually or as clinically indicated, for patients with smoldering myeloma and Bence Jones proteinuria 5
