Metronidazole Use During Pregnancy

Safety Profile

  • Metronidazole can be safely used during pregnancy when clinically indicated, as it is not associated with increased risk of congenital malformations, premature births, stillbirths, spontaneous abortions, or low birth weight, according to the CDC and Gastroenterology guidelines 1, 2
  • Current guidelines support the use of metronidazole for specific indications during pregnancy, including trichomoniasis, bacterial vaginosis, and certain gastrointestinal infections, as recommended by the CDC and Gastroenterology 1, 2

Specific Indications

  • For trichomoniasis during pregnancy, the CDC recommends a single 2g dose of metronidazole 1
  • For bacterial vaginosis in high-risk pregnant women, treatment is recommended in the early second trimester, as suggested by the CDC 3
  • For inflammatory bowel disease complications, metronidazole can be safely administered during pregnancy, according to Gastroenterology guidelines 2, 4

Important Precautions

  • Patients should be advised to avoid alcohol during treatment with metronidazole and for 24 hours afterward due to potential disulfiram-like reactions 5
  • For patients with documented allergy to metronidazole, desensitization under medical supervision is recommended as the first-line approach, as suggested by medical guidelines 6
  • Metronidazole gel is not recommended for trichomoniasis treatment during pregnancy as it achieves cure rates of less than 50% compared to 90-95% with oral therapy, according to medical guidelines 5, 6

Special Considerations

  • Treatment of sexual partners is essential to prevent reinfection when treating sexually transmitted infections, as recommended by medical guidelines 6
