Off-Label Uses of Gabapentin

Neuropathic Pain Conditions

  • The American Academy of Ophthalmology recommends gabapentin as a third-line agent for neuropathic corneal pain, with evidence showing significant decrease in pain compared to placebo in post-herpetic neuralgia 1
  • Gabapentin is frequently used off-label for neuropathic corneal pain at doses >1200 mg/day in divided doses, with significant decrease in pain compared to placebo 1

Gastroparesis Management

  • The American Gastroenterological Association recommends gabapentin for visceral pain in gastroparesis at doses >1200 mg daily in divided doses, with evidence showing >50% reduction in neuropathic pain compared to placebo 2, 3, 6
  • Gabapentin is listed as an anticonvulsant option for pain management in gastroparesis in clinical practice guidelines, with evidence showing >50% reduction in neuropathic pain compared to placebo 2, 3, 6

Opioid Withdrawal Management

  • The American Society of Addiction Medicine suggests gabapentin as an adjunctive medication during opioid tapering to mitigate withdrawal symptoms including anxiety, insomnia, and irritability 4, 5
  • Gabapentin can help manage symptoms that don't respond to medications that directly counteract physiologic changes of opioid withdrawal, with evidence from clinical trials 4, 5

Dosing Considerations

  • For neuropathic pain conditions, doses typically range from >1200 mg/day in divided doses, with significant decrease in pain compared to placebo 1, 2
  • For gastroparesis-related pain, doses >1200 mg daily in divided doses are recommended, with evidence showing >50% reduction in neuropathic pain compared to placebo 2, 6
