Anticoagulation Management in Patients with Chronic Kidney Disease Undergoing Surgery
Introduction to Anticoagulation Therapy
- The American College of Chest Physicians recommends using unfractionated heparin (UFH) or low molecular weight heparin (LMWH) with dose adjustment according to renal function for perioperative thromboprophylaxis in patients with chronic kidney disease (CKD) undergoing elective surgery, avoiding direct oral anticoagulants (DOACs) in stage 4-5 CKD, and reserving well-controlled warfarin (INR 2.0-3.0, TTR >65-70%) for therapeutic anticoagulation in patients with advanced CKD requiring long-term treatment 1, 2
Stratification According to Renal Function
- For patients with mild CKD (stage 2, CrCl 60-89 mL/min), anticoagulation decisions should follow the same recommendations as for patients without CKD 2
- For patients with moderate CKD (stage 3, CrCl 30-59 mL/min), use DOACs with dose adjustment according to label or warfarin with strict control (TTR >65-70%) 2
- For patients with severe non-dialytic CKD (stage 4, CrCl 15-30 mL/min), use warfarin as first-line therapy with strict monitoring of INR (target 2.0-3.0, TTR >65-70%) 2, 1
- For patients with end-stage renal disease (stage 5, CrCl <15 mL/min or dialysis), well-controlled warfarin (INR 2.0-3.0, TTR >65-70%) is the anticoagulant of choice for therapeutic treatment 2, 1, 5
Perioperative Management
- For patients undergoing high-risk bleeding surgery, suspend DOACs 4-5 days before the procedure if CrCl >50 mL/min 4
- For patients with high thromboembolic risk (mechanical valve, CHA₂DS₂-VASc ≥5-6, recent stroke <3 months), consider bridging therapy with UFH or LMWH 7, 2
- Reinitiate therapeutic anticoagulation 24-72 hours postoperatively once surgical hemostasis is achieved 4
Considerations and Pitfalls
- Calculate creatinine clearance using the Cockcroft-Gault formula, not eGFR, for anticoagulant dosing 4
- Avoid concomitant antiplatelet therapy, including low-dose aspirin, in patients with CKD initiating anticoagulation, as it substantially increases the risk of bleeding 2, 6
- Inhibitors of P-glycoprotein increase concentrations of all DOACs, and inhibitors of CYP3A4 increase concentrations of factor Xa inhibitors (rivaroxaban, apixaban, edoxaban) 4
Decision Algorithm
- For perioperative thromboprophylaxis, evaluate CrCl (Cockcroft-Gault) and bleeding risk of the procedure 4
- If CrCl >50 mL/min, use UFH, LMWH without dose adjustment, or apixaban 2.5 mg twice daily 3
- If CrCl 30-50 mL/min, use UFH or LMWH with dose adjustment (preferably tinzaparina) 3
- If CrCl <30 mL/min, use UFH preferentially 3
